Plastic Surgery

  1. ONE DAY PRIOR SURGERY, YOUR COORDINATOR WILL LET YOU KNOW THE TIME YOU HAVE TO BE AT THE HOSPITAL (IT COULD BE FROM 6:00 A.M. TO 3:00 P.M.). LIABLE TO CHANGES .
  2. THE DEPOSIT IS NOT REFUNDABLE UNDER ANY CIRCUMSTANCE, NO EXCEPTIONS.
  3. THE MEDICAL FILE MUST BE COMPLETED CORRECTLY RIGHT AFTER YOUR SEND THE DEPOSIT, IN CASE OF NOT BE COMPLETED THE COORDINATOR WILL PAUSE THE PROCESS OF BOOKING THE SURGERY AND IT WILL BE LIABLE TO CHANGES.
  4. IF THE PATIENT ADDS EXTRA PROCEDURES ON THE MEDICAL FILE THAT WERE NOT MENTIONED ON THE QUOTE, THE COST NEEDS TO BE COVERED BY THE PATIENT.
  5. WE DO REQUIRE A PICTURE OF OFFICIAL ID NON EXPIRED.
  6. IN CASE THAT THE PRE OP STUDIES SHOWS A RESULT THAT THE PATIENT’S HEALTH IS NOT IN THE ACCEPTED PARAMETERS FOR PLASTIC SURGERY, THE INTERNIST DOCTOR WILL BE RESPONSIBLE FOR DECIDING WHETER THE SURGERY MAY CONTINUED OR WILL BE CANCELED/RESCHEDULED, LIABLE TO AVAILABILITY AND THE DEPOSIT WILL BE LOST.
  7. CHANGES OF DATE, PROCEDURE AND/OR SURGEON MADE BY THE PATIENT MAY AFFECT THE ORIGINAL QUOTE.
  8. VIDEOS AND/OR PICTURES OF BEFORE AND AFTER SURGERY CAN BE USED BY VIVE PLASTIC WITH MARKETING/ADVERTISING PURPOSES .
  9. IS THE PATIENTS RESPONSIBILITY TO GET A PREGNANCY TEST DONE BEFORE SURGERY. IF THE DOCTOR DETERMINES THAT THE PATIENT IS PREGNANT, THE SURGERY WILL BE CANCELED LOSING THE DEPOSIT.
  10. IN CASE OF EXPERIENCING SYMPTOMS OF ANY SICKNESS IS THE PATIENT’S RESPONSIBILITY TO ADVISE THE COORDINATOR, THE CASE WILL BE ESCALATED TO THE INTERNIST DOCTOR WHO IS GOING TO PROVIDE INSTRUCTIONS ON HOW TO TREAT THE MENTIONED SICKNESS.
  11. NOT ANSWERING 3 DAYS IN A RAW THE CALLS FROM THE COORDINATION TEAM IS GOING TO BE CATALOGED AS BAD COMMUNICATION AND POSSIBLE CANCELATION LOSING THE TOTAL OF YOUR DEPOSIT. NO EXCEPTIONS.
  12. IS THE PATIENTS RESPONSIBILITY BRING EXTRA CASH FOR ANY FEES PREVIOUSLY MENTIONED IN THE CONFIRMATION EMAIL, AS WELL, READ AND FOLLOW ALL THE INSTRUCTIONS PROVIDED BY VIVE PLASTIC SURGERY TEAM.
  13. NOT FOLLOW THE INSTRUCTIONS FROM THE COORDINATORS MAY AFFECT YOUR HEALTH BEFORE, DURING AND/OR AFTER SURGERY, EVEN A POSSIBLE CANCELATION OF YOUR SURGERY LOSING THE TOTAL OF YOUR DEPOSIT.
  14. IF THE PATIENT DOES NOT ADVISE CHANGES OF DATE 30 DAYS PRIOR SURGERY WITH THE COORDINATOR & TO THE EMAIL VIVEPLASTICSURGERY@GMAIL.COM THE TOTAL OF YOUR DEPOSIT WILL BE LOST.
  15. THE DATE & HOUR ARE LIABLE TO CHANGES BY EXTRA-ORDINARY EVENTS, WE ARE NOT RESPONSABLE OF CHANGES/FEES ON FLIGHTS, TRANSPORTATION OR EXTRA SERVICES HIRE BY THE PATIENTS.
  16. IN CASE OF CANCELLATION IF IT IS PRIOR 30 DAYS BEFORE SURGERY, THE PATIENT CAN USE THE 100% OF THE DEPOSIT AS CREDIT ON VIVE MEDICAL SPA & IT HAS AN EXPIRATION OF 1 MONTH, PATIENT NEEDS TO SET UP DATE AND HOUR FOR THE PROCEDURE.
  17. DEPOSIT IS NOT TRANSFERABLE TO THIRDS.
  18. THE PRICE OF SURGERY DOES NOT INCLUDES EXTRA GIRDLE/FAJA, MEDICATION, RECOVERY HOUSE, MASSAGES OR TRANSPORTATION, EVERY SERVICE HAS THEIR OWN POLICIES & RESTRICTIONS, IS THE PATIENT’S RESPONSIBILITY TO COVER THE COSTS OF EVERY SERVICE IF THEY ARE REQUESTED.
  19. IS THE PATIENTS RESPONSIBILITY GET PRE-OP STUDIES BEFORE SURGERY, IF THE RESULTS ARE NOT READY 4 WEEKS PRIOR THE DATE OF SURGERY THIS WILL BE LIABLE TO CHANGES OR CANCELATION WITH PREVIOUS ADVISE AND DEPOSIT WILL BE LOST.
  20. THE REMAINING TO PAY IS EXCLUSIVELY IN CASH OR CASHIERS CHECK, WE DO NOT TAKE ANY OTHER PAYMENT METHODS.
  21. PAYMENTS WITH PAYPAL ARE EXCLUSIVELY FOR DEPOSITS, NOT TO COVER THE TOTAL PRICE OF SURGERY. IN CASE THAT THE PATIENT DOES NOT HAVE THE REMAINING TO PAY, THE SURGERY WILL BE CANCELED AUTOMATICALLY WITHOUT REIMBURSEMENT OF DEPOSIT. THE MENTIONED DEPOSITS AND/OR PAYMENTS WILL BE VALID UNTIL A CONFIRMATION FROM THE ACCOUNTANT & IS PATIENT’S RESPONSIBILITY ADVISE YOUR COORDINATOR EVERY SINGLE TIME THE PATIENT MAKES A MOVEMENT OR TRANSACTION, AS WELL SEND A VOUCHER OR RECEIPT CLEAR AND DETAILED WITH THE REFERENCE NUMBER, NAME OF BENEFACTOR & BENEFICIARY.
